Richland County restaurant inspections: critical violations Sept. 22-24.

These restaurant inspections with critical violations were conducted by Richland Public Health between Sept. 22 and Sept. 24:

● Subway Sandwich Shoppe #38123, 359 N. Lexington Springmill Road, Ontario, Sept. 22. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Education provided for critical violations during inspection. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). Employee unable to locate employee illness reporting agreements. Richland Public Health's (RPH) reporting agreement handout provided. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed dried lettuce on spatula in cleaned/sanitized utensil storage near three-compartment sink.

Improper use of a poisonous or toxic substance (critical, corrected during inspection). Observed spray can of Hot Shot spray stored on a shelf next to packages of single use condiments, as well as a can of Raid sitting on table where boxes of chips. PIC discarded cans of pesticide during inspection. Presence of live insects (critical). Observed small flying insects near fountain beverage station, back storage room and near mop sink. Remove all presence of small flying insects or contact licensed pest control company for treatment by Sept. 29.

● Rama Tika Managment LLC, DBA 24 Store, 1316 Ashland Road, Mansfield, Sept. 22. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed clear liquid stored in a bottle located on front counter.

● Bob Evans Restaurants LLC #219, 30 W. Hanley Road, Mansfield, Sept. 22. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Two pitchers of tea observed unattended, uncovered and subject to sneeze, splash, dirt and dust. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Cold holding Dellfield had french fries and chicken strips observed in containers with no lids. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer bucket on registered with no sanitizer in it. Improper cooling of TCS food (critical repeat).

Employee was not beginning cooling process at 160 degrees and not 135 degrees. Educational handout and cooling chart provided to PIC. Correct by Sept. 30. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Chicken gravy measured only 124 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Egg whites observed stored in cooler for over 24 hours without proper datemarking. Ready-to-eat, TCS food not properly discarded when required (critical). Gallon of opened white milk observed that had expired on Sept. 21. PIC voluntarily discarded.

Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Ham was observed expired on Sept. 19. PIC voluntarily discarded listed item. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Sausage observed without proper date-marking. PIC voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Bacon pieces observed stored in walk-in cooler that had expired on Sept. 20, 2021. PIC voluntarily discarded.

● Plymouth High School Concession Stand, 400 Trux St., Plymouth, Sept. 23. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Draft agreement given to PIC; workers reviewed and signed report. Food employee(s) did not wash hands when required (critical, corrected during inspection). Worker observed donning new gloves before washing hands. TCS foods not being cold held at the proper temperature (critical, corrected during inspection).

RTE lettuce cold holding was measured at 54 degrees. PIC voluntarily discarded. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Hornet spray bottles observed below three compartment sink. Listed items were removed from premises. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Wash solution measured at only 87 degrees.

● WalMart #1539, 359 N. Lexington Springmill Road, Mansfield, Sept. 23. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed uncooked salt pork stored above fully cooked turkey drumsticks in retail meat case. Also observed raw ground pork (cook temperature 155 degrees) stored above raw pork roasts (cook temperature 145 degrees) in retail meat case. Unsafe, adulterated or not honestly presented food not discarded or properly reconditioned (critical, corrected during inspection). Observed multiple cans on shelving in retail area with dents along seals. PIC removed cans from retail area for discard.

Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed two blocks of opened pepper jack cheese that exceeded datemark for discard of Sept. 18 and Sept. 21. Employee discarded cheese. Discussed ensuring any cheeses not exempted from date marking are properly date marked for a consumption or discard within seven total days once opened. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed scissors in deli area used to cut open bags of frozen chicken not able to be fully separated and taken apart for effective cleaning. Water source and/or hot water capacity not sufficient (critical). Observed hot water at three-compartment sink in deli area to only be at 108 degrees. Ensure hot water is at a minimum of 110. Correct by Sept. 30.

● The Food Tour, 2253 Satinwood Drive, Mansfield, Sept. 24. Food employee(s) did not wash hands when required (critical, corrected during inspection). Food worker observed donning new gloves without first washing hands.

● Cruizin Smoothies, 295 E. Ohio 96, Shiloh, Sept. 24. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand wash sink observed in use for storage.
