Marion County Restaurant Inspections: Chen's, Cali Tacos, Rudy's Encore

This is a snapshot of the inspections conducted by the Marion County health department. 

Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness. 

Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.  

Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

Food-contact surfaces of equipment and utensils are not sanitized after cleaning, specifically: Blender and lid were washed and rinsed and then placed on shelf to air dry with no sanitizing step. Point deduction: 5.

Location: 1560 N. Pacific Highway, Woodburn

Location: 2138 Lancaster Drive NE, Salem

A handwashing sink is not accessible for employee use at all times, is used for purposes other than handwashing or is not operated properly, specifically: The cookline handwashing sink is blocked by colanders. Point deduction: 3.

Raw or ready-to eat food is not properly protected from cross contamination, specifically: In the prep cooler, raw scallops and shrimp are stored on the upper shelf with cooked noodles and mushrooms below. In the upright display cooler, raw pork is stored above raw cut cabbage. In the upright true freezer, many white grocery-style bags of raw meat are stored above or touching ready to eat foods like egg rolls or foods with lower cooking temperatures. Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Milk inside the small reach-in espresso refrigerator is 47 degrees Fahrenheit. Point deduction: 5.

Working containers of poisonous or toxic materials are not clearly labeled, specifically: A spray bottle of cleaner is not labeled. Point deduction: 3.

The chlorine sanitizer concentration, pH, or temperature is not adequate, specifically: Observed chlorine concentration at 0 parts per million in the dishwasher during today's inspection. Point deduction: 5.

Location: 2280 Broadway St. NE, Salem

Food employees are not washing their hands as often as necessary, specifically: Observed pic go out of mobile to turn off the water faucet, and then rinse hands in the sink before returning to work. Point deduction: 5.

Raw or ready-to eat food is not properly protected from cross contamination, specifically: Although the raw meat is stored on the bottom shelf of the residential refrigerator, the produce drawer is in use directly underneath. Point deduction: 5.

Temperature measuring devices are not provided or readily accessible for checking food temperatures, specifically: The food thermometer does not measure temperatures below 50 degrees Fahrenheit. Point deduction: 3.

A test kit is not provided or is not accurate enough to measure the concentration of sanitizing solutions, specifically: Chlorine sanitizer is present, but there are no test strips available. Point deduction: 3.

The chlorine sanitizer concentration, pH, or temperature is not adequate, specifically: Measured 0 parts per million chlorine in dish machine. Sanitizer dispenser is empty. Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Measured half & half at 45 degrees Fahrenheit in under counter reach-in. Ambient temperature is measured at 49 degrees. Observed ice build-up in back of fridge. Point deduction: 5.

Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: Tomato juice dated Aug. 29 and half & half opened four days ago are past the seven day disposition or are not properly date marked. Point deduction: 5.

Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: Items not properly dated in prep line including potato salad with old date mark from July, macaroni salad missing date, macaroni salad dated Sept. 6. Point deduction: 5.

A consumer advisory is not provided, is not presented in the correct format, or does not contain the required information, specifically: Eggs and burgers served to order are missing asterisk linking items to printed consumer advisory on menu. Point deduction: 3.

Location: 3890 Center St. NE, Salem

The chlorine sanitizer concentration, pH, or temperature is not adequate, specifically: No chlorine dispensing from the dishwasher during the sanitizing cycle; chlorine residual measured 0 parts per million. Point deduction: 5.

Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Chocolate milk at 52 degrees Fahrenheit, milk at 49 degrees and yogurt at 48 to 49 degrees at the front display beverage refrigerator. Point deduction: 5.

Working containers of poisonous or toxic materials are not clearly labeled, specifically: Four unlabeled spray bottles observed in the back storage area. Point deduction: 3.
